Who needs a rail medical?

Anyone performing safety-critical work — such as train drivers, track workers, shunters, or signallers — must undergo periodic rail medicals in line with rail standards.

How often do rail medicals need to be done?

Rail medical review frequency depends on the employer, role, and applicable standard.

Train Drivers: Typically 3-yearly until age 53, then at the 56th birthday, and annually thereafter.
Network Rail: Reviews every 10 years up to age 39, 6-yearly from 40–49, 4-yearly from 50–59, and 2-yearly from age 60.
Train Movement roles (RIS-3789-TOM): Age-dependent reviews following 10, 6, 4, and 2-yearly intervals.
London Underground (LUL): Uses specific medical standards and review frequencies which differ from Network Rail and RIS-3789-TOM.

Can any doctor carry out rail medicals?

Yes, however only ORR Recognised Doctors or LUL Track Approved certified doctors listed on the official register can oversee and sign off the train driver and safety critical worker medicals. Our assessments are led by Dr Sarwar Chowdhury (ORR No. 156).

What happens during a rail medical?

The medical includes a history review, physical examination, vision testing, hearing testing, colour vision, fatigue screening, ECGs for Train Drivers and may include drug and alcohol testing depending on role. All assessments follow ORR and Railway Group Standards.

Can you deliver on-site medicals for groups?

Yes. We regularly deliver on-site clinics for rail contractors, TOCs and FOCs. This minimises operational downtime and ensures compliance for large workforces.
