What is a HAVS Medical and Why is it Required?

A HAVS Medical (Hand Arm Vibration Syndrome Medical) is a statutory health surveillance process for employees who are exposed to hand-transmitted vibration in the workplace. Regular use of vibrating tools, machinery, or equipment can lead to Hand Arm Vibration Syndrome (HAVS), Carpal Tunnel Syndrome or other vibration related conditions that may cause permanent damage to blood vessels, nerves and joints in the hands and arms.

Under the Control of Vibration at Work Regulations 2005, employers have a legal duty to provide tiered health surveillance for workers exposed at or above the ‘Action Value’. Health surveillance must be carried out by occupational health professionals trained in HAVS and overseen by a qualified occupational health doctor.

The goal of a HAVS medical is early detection of symptoms, ensuring workers remain safe to carry out their duties supporting employers to remain compliant with Health and Safety Executive (HSE) requirements.

Tiers 1 & 2: Screening

Initial and annual questionnaires to identify early symptoms before or during employment.

Tier 3: Clinical Assessment

Structured clinical exam of circulation and sensation by a qualified OH professional.

Questions

At Work Doctor OH, we deliver HAVS health surveillance in line with the HSE’s tiered approach. Depending on the stage, the medical may include:

  • Health Questionnaires: Tiers 1 and 2 screening.
  • Clinical Assessments: Tiers 3 and 4 combined assessment.
  • Specialist Referral: Tier 5 assessment if clinically indicated.
  • General Health Checks: Including BMI, circulation, and neurological function (fine touch, manual dexterity, grip strength, and vasculature/blood vessel examinations).

Following assessment, a Fitness for Work Certificate is provided to the employer. Where restrictions or adjustments are required, clear advice is given.

A GP’s sick note (Fit Note) focuses on the employee’s health condition and states whether they are “fit for work” or “not fit for work.” There is limited advice on adjustments for work or phased returns. An occupational health referral provides an expert, detailed independent opinion on the employee’s health in relation to their specific job, offering practical advice and recommendations for the employer to consider.
  • A Tier 1 baseline questionnaire before exposure.
  • Annual Tier 2 screenings thereafter.
  • Escalation to Tiers 3–5 only if symptoms are reported.

Workers may be referred to a higher Tier assessment. Employers may need to reduce exposure or adjust duties.
The employer is responsible for providing and funding all required HAVS health surveillance.
