What is a Lead Medical and Why is it Required?

A Lead Medical is a statutory health assessment for employees who are significantly exposed to lead or its compounds in the workplace. The medical is designed to monitor for any adverse health effects from lead exposure and to ensure that workers are fit to continue their duties safely. Under the Control of Lead at Work Regulations 2002 (CLAW), employers must provide medical surveillance for employees who are significantly exposed to lead. This includes anyone likely to inhale lead dust, fumes, or vapour, or ingest lead through occupational activities such as smelting, battery manufacturing, paint stripping, or construction work involving old lead-based materials.

Your lead medical assessment begins with a medical questionnaire, followed by a clinical consultation and a blood-lead level test. Once complete, we provide a fitness certificate and health record that confirms your suitability for work, outlines any necessary adjustments, and specifies your next review date.

Questions

At Work Doctor OH, lead medicals are carried out by appointed doctors, as required if the exposure to lead is deemed ‘significant’ after an appropriate risk assessment. The assessment typically includes:

  • Medical & Occupational History – including details of lead exposure, previous health issues, and lifestyle factors.
  • Physical Examination – where appropriate, to assess overall fitness to work with lead.
  • BMI Check – height, weight, and body mass index measurement.
  • Biological Monitoring – employees working with lead are required to undergo regular tests to measure blood-lead levels (arranged in conjunction with the employer). This may be a blood test (inorganic lead) or urine test (organic lead) testing
  • Discussion of Results – advice and recommendations based on exposure and test outcomes.

After the medical, the employee will be issued with a Certificate of Fitness for Work with Lead, valid until the next scheduled review.

We can also provide group anonymised lead worker data and outcomes to the employer as required by CLAW

Employees who are exposed to significant levels of lead in their work, as defined by the Control of Lead at Work Regulations 2002, must undergo medical surveillance. This applies to industries such as smelting, battery manufacturing, foundries, and construction. We can also provide health and biological monitoring for workers with ‘non significant exposures’ if required.

An initial medical is required before an employee starts work with lead.

Ongoing medicals are usually carried out annually, though frequency may vary depending on blood-lead results and doctor’s recommendations.

This involves taking blood samples to measure the concentration of lead in the body. Results help determine whether an employee can continue working with lead or needs to be temporarily removed from exposure.

If blood-lead levels exceed HSE action limits, the worker must be temporarily suspended from further lead exposure until levels return to safe limits.
