What is an Ionising Radiation Medical and Why is it Required?
An Ionising Radiation Medical is a statutory health assessment for workers who are classified as radiation workers under UK law. It ensures individuals exposed to (or having the potential to be significantly exposed to) ionising radiation (such as X-rays, radioactive materials, or nuclear industry sources) are medically fit to undertake their work and remain under appropriate health surveillance.
Under the Ionising Radiations Regulations 2017 (IRR17), employers must arrange for classified radiation workers to undergo a medical examination by an HSE-appointed doctor. The medical assessment is required on initial classification and must be repeated at least once every 12 months (or sooner depending on the medical recommendations). This involves face to face assessments initially and every 5 years with ‘paper reviews’ every year in between

What Does the Ionising Radiation Medical Involve?
At Work Doctor OH, all radiation medicals are conducted by experienced, doctor-led occupational health professionals who are appointed under IRR17. The assessment typically includes:
- Comprehensive Medical & Occupational History – including previous radiation exposure, general health, and relevant lifestyle factors.
- Physical Examination – if required focusing on fitness for radiation work.
- Additional Tests (if required) – further investigations may be requested depending on medical history or exposure risks.
Following the assessment, a Certificate of Fitness to Work with Ionising Radiation will be issued, valid for up to 12 months, in line with IRR17 requirements.
Who needs an ionising radiation medical?
Any worker classified as a radiation worker under the Ionising Radiations Regulations 2017 must undergo a medical examination by an HSE-appointed doctor.
Who is defined as a radiation worker?
A radiation worker (classified person) is someone who, due to their work, meets any of the following criteria:
- Likely to receive an effective dose of >6 mSv of radiation per year.
- Likely to receive more than three-tenths of any relevant dose limit.
- Has the potential to be exposed to these levels in the event of an accident or over-exposure.
This classification triggers formal medical surveillance by an appointed doctor, statutory record keeping, and stricter legal controls.
How often is the medical required?
An initial medical is required before classification, followed by annual reviews (every 12 months). The appointed doctor may specify shorter intervals if needed.
What happens if my employees don’t have a valid certificate?
Employers would be in breach of IRR17. The HSE could take enforcement action, including fines, suspension of work, or prosecution.
What certificate will I receive?
You will be issued with a Radiation Fitness Certificate, confirming whether you are fit, fit with restrictions, or unfit for radiation work.
How long does the medical take?
Most radiation medicals take 20–30 minutes, depending on the individual’s medical history and whether additional tests are needed.
Is the cost covered by the employer?
Yes – the employer is legally responsible for the cost of medical surveillance for radiation workers. This should be within working hours
